The results of the research are described in ScienceDaily, but here are the key findings.

The first study, based on a genetic analysis of more than 9,000 people, has found that certain versions of four genes may speed shrinkage of a brain region involved in making new memories. The brain area, known as the hippocampus, normally shrinks with age, but if the process speeds up, it could increase vulnerability to Alzheimer’s disease, the research suggests.

Apparently bigger is better where it concerns the hippocampus.

The gene variants identified in the first study do not cause Alzheimer’s, but they may rob the hippocampus of a kind of “reserve” against the disease, which is known to cause cell destruction and dramatic shrinkage of this key brain site. The result is severe loss of memory and cognitive ability.

Scientists calculated that hippocampus shrinkage in people with these gene variants accelerates by about four years on average. The risk of Alzheimer’s doubles every five years beginning at age 65, so a person of that age would face almost twice the Alzheimer’s risk if he or she had these versions of the gene.

Looked at another way, if a person with one of these variants did get Alzheimer’s, the disease would attack an already compromised hippocampus and so would lead to a more severe condition at a younger age than otherwise, the research suggests.

“This is definitely a case of ‘bigger is better,’” said DeCarli. “We already know that Alzheimer’s disease causes much of its damage by shrinking hippocampus volume. If someone loses a greater-than-average amount of volume due to the gene variants we’ve identified, the hippocampus is more vulnerable to Alzheimer’s.”

Why the aging hippocampus normally decreases in volume is unclear.

How to Recognize the Symptoms of Alzheimer’s Disease

Alzheimer’s disease is a particular type of dementia that causes a progressive loss of intellectual and social skills severe enough to interfere with day-to-day life. With Alzheimer’s disease, brain cells degenerate and die, causing a steady decline in memory and mental function.

The first symptoms of Alzheimer’s disease are increasing forgetfulness and mild confusion. Some people notice difficulty remembering things and organizing their thoughts, while others may not recognize that anything is wrong. Over time, memory is impaired and, as the disease progresses, the individual may develop problems speaking, writing, solving problems and making sound judgments.

People with Alzheimer’s also suffer problems with orientation and interpreting spatial relationships. They may lose their sense of what day it is, the time of year, where they are or even their current life circumstances. Alzheimer’s also may disrupt the brain’s ability to interpret what you see, making it difficult to understand your surroundings. Eventually, these problems may lead to getting lost in familiar places. This is one of the reasons folks with Alzheimer’s should not drive.

Responding effectively to everyday problems, such as food burning on the stove or unexpected driving situations, becomes increasingly challenging. Planning and performing familiar tasks becomes a problem. Once-routine activities that require sequential steps, such as planning and cooking a meal or playing a favorite game, become a struggle as the disease progresses. Eventually, people with advanced Alzheimer’s may forget how to perform basic tasks such as dressing and bathing.

Family members also notice changes in personality and behavior. People with Alzheimer’s disease may experience depression, anxiety, social withdrawal, mood swings, distrust in others, increased stubbornness, irritability/aggressiveness, changes in sleeping habits and wandering.

Ineffective Alzheimer’s Treatment

Unfortunately, it appears that Aricept 23 mg is an ineffective Alzheimer’s treatment. When it comes to memory improvement and Alzheimer’s treatment options, family members and caregivers often reach out to the newest hope — medication that will stop or even reverse the effects of this devastating disease. Whether or not they actually improve memory is hard to measure. However, some can have negative side effects.

Drs. Steven Woloshin and Lisa Schwartz of Dartmouth Medical College are authors of a report that describes the negative side effects of  Aricept 23 mg.

Ineffective Alzheimer’s Treatment

The drug, Aricept 23 mg, is no more effective on the whole than the disappointing ones already on the market — but is more likely to cause gastrointestinal problems, wrote Drs. Steven Woloshin and Lisa Schwartz of Dartmouth Medical College in an article published Thursday in the medical journal BMJ.

In a trial involving 1,400 patients with moderate to severe Alzheimer’s, the 23-milligram dose of Aricept resulted in a small but statistically significant improvement in an index of overall cognition. But on a second measure, of “global functioning” — changes in behavior that a caregiver or physician is likely to notice — it failed to yield the improvements that the FDA had set as a condition of approval.

At the same time, subjects taking the 23-milligram dose, when compared with those taking 10 milligrams, reported significantly more nausea and vomiting — described by the director of the FDA’s neurological drug division as “not trivial.” In patients with dementia, nausea and vomiting can lead to pneumonia and death.

Dr. Lon Schneider, a USC Alzheimer’s disease expert, said there had been widespread interest in whether a 23-milligram dose might help the minority of Alzheimer’s patients taking two 10-milligram pills daily. Physicians, he said, hoped that one higher-dose pill would not only improve patients’ dementia symptoms, but also release more slowly into the bloodstream, causing less stomach upset.

Dementia is a sad enough disease without making it physically worse for the sufferer and the caregivers. I’ve watched the decline of several family members, and my opinion was not to do anything that made the overall quality of life worse.
